(Toledo News Now) - A sushi restaurant racks up more than a dozen violations in this week's Restaurant Ratings Report -- and almost half of them are repeats.

Osaka Sushi and Hibachi on Navarre Avenue in Oregon received 13 violations during its latest inspection, including improper food storage, utensils stored in standing water, and a blocked hand sink -- all of which are repeat violations. Osaka was also cited for raw chicken being prepared next to zucchini and unsafe food storage temperatures.

Charlie's Restaurant Taverna on Toll Gate Drive in Maumee was also cited for unsafe food storage temperatures after inspectors noticed issues with raw beef patties, raw eggs, and spinach pie. There were seven violations in all, including food stored on the floor in the walk-in freezer and a lack of date markings on food.

Inspectors tallied seven violations at Little Caesar's Pizza on West Alexis Road in Toledo last month, including broken storage containers and a leaking sink.

Unsafe food storage temperatures could be found at Mama Mary's Pizza along Airport Highway in Holland. The chlorine concentration in the sanitizer was 0 parts per million, when it should be kept at 50 to 100 parts per million.

Wonder Meal on Lewis Avenue in Toledo is also having problems with chlorine concentration in the sanitation bucket. The restaurant was cited for four violations, including expired grape leaves and gyro meat stored at an unsafe temperature.
