Registration Process:
All children, regardless of race, color, religion, sex or national origin, are welcome at The Gathering Place. The following information is needed to enroll your child:

- Completed registration forms
- The prescribed Child Enrollment and Health Information form must be completed for the first day of attendance ( State PDF form )
- Immunizations record on file in the Center's office ( State PDF form )
- Permission to transport to a medical center
- Physician signed and dated medical form (must be submitted within 30 days of enrollment) ( State PDF form )
- Completed EFT form or credit card information
- Paid, non-refundable registration fee
- Paid, non-refundable material fee
- First week's tuition
- Income eligibility applicaton
- Copy of child's birth certificate, custody papers, etc., as needed
