Police say 68-year-old Phyllis Marinoff of Clyde was westbound on US-20 and slowed for traffic. That is when 57-year-old Stephen Paine of Toledo, Ohio failed to stop and struck her from behind.
The impact caused Marinoff to go across the center median into the eastbound lanes.
Thirty-nine-year-old Joseph Sultana of Garden City, Michigan was going eastbound in a semi when he swerved in an attempt to miss Marinoff, but the trailer of the semi completely rolled over her vehicle.
The trailer then went airborne onto a car driven by 40-year-old Adam Hellman of Fremont, Ohio. His vehicle went off the right side of the road, striking a sign before coming to a stop in a small ditch.
Marinoff was pinned in the vehicle and had to be removed by the "Jaws of Life." She was taken by Life Flight and flown to the Mercy Saint Vincent's Hospital with serious injuries.
Paine, Saltana and Hellman were not injured and required no treatment.
Alcohol is not believed to be a factor.
The crash remains under investigation.
